Doctor Cowboy was the imaginary friend of Victor Stone. It has the appearance of his father.
The Candlemaker made him appear to Vic in the fair.

When Victor Stone was a child, his father Silas did not have much time for him so Vic chose to create an imaginary friend like his father calling him Dr. Cowboy to give him advice and support him in whatever but to move on from Vic grew older and began to forget his imaginary friend.
Reunion with Vic[]
When the Doom Patrol arrived at the fair, Dr. Cowboy was one of the imaginary friends that the Candlemaker summoned to confront the Doom Patrol. Vic was monitoring the playground so he could find Niles and Dorothy but Dr. Cowboy surprised him. Doctor Cowboy was impressed by the cybernetic parts of Vic and challenged him to an old west style shootout. Vic shot first defeating Cowboy and then turned, thinking he had won. Dr. Cowboy revived and told Vic they needed to talk. The two spoke about Roni and Vic confessed everything to him; how he wanted Roni to be a good person but it all ended badly. Dr. Cowboy was surprised by the whole relationship that Roni and Vic had. Cowboy gave Vic some advice to get over Roni before the Candlemaker took control of Cowboy and told Vic that he could never accomplish anything and that he was incapable of love. Vic was blindsided by the Candlemaker and then waxed before he could escape.
